Carease Health in Illinois is hiring a Nurse Practitioner (NP) or Physician Assistant (PA) for a hybrid role combining on-site visits at skilled nursing facilities with remote work from home. You will join a proactive clinical team delivering preventative care using RPM, CCM, and CoCM to help reduce hospitalizations and elevate CMS ratings.

The position offers a flexible schedule, ongoing training, and a supportive environment with opportunities for professional growth.

Qualificações

• Active NP or PA license in the state of practice.
• 1+ year of experience in geriatric care and documentation review.
• Excellent communication, collaboration, and teamwork skills.

Responsabilidades

• Provide high-quality preventative care to residents in skilled nursing facilities
• Conduct on-site visits and remote work from home, supported by a scribe team for efficient documentation
• Build relationships with facilities while maintaining a patient-centered approach and being a collaborative team player
• Stay up-to-date with industry trends and best practices, attending regular training sessions and conferences
