Carease Health - IL is seeking a Nurse Practitioner or Physician Assistant to provide preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

The position is part-time (approximately 9 hours per week) with opportunities for professional growth, training from clinical directors, generous PTO, health insurance for full-time employees, free CEUs, and referral

Qualifications

• Active NP or PA license in the state of practice.
• 1+ year of experience in Geriatric care and documentation review.
• Excellent communication, collaboration, and teamwork skills.

Responsibilities

• Provide high-quality preventative care to residents in skilled nursing facilities.
• Conduct on-site visits and remote work from home, supported by a dedicated scribe team.
• Build strong relationships with facilities and maintain a patient-centered approach.
• Collaborate with the clinical team to ensure seamless integration with facility processes and protocols.
