Job summary
Strive Health is hiring a Nurse Practitioner to lead interdisciplinary teams in delivering longitudinal, home- and telehealth-based care for patients with CHF, CKD/ESKD and other chronic conditions. You will employ advanced analytics and cross-disciplinary collaboration to provide high-quality care wherever the patient is located.
The role emphasizes reducing hospitalizations, optimizing care transitions, and embracing a whole-person approach with strong patient engagement.
Qualifications
- Master's degree in Nursing or similar field.
- Current state specific licensure.
- National Board Certification.
- Current DEA license or eligible to obtain within 90 days of hire.
- Current BLS or CPR Certification.
- Efficient and reliable transportation with active driver's license for travel across region.
Responsibilities
- Manages a defined patient panel with accountability for quality, cost and outcomes; collaborates with external providers.
- Obtains patient history, performs physical exams, orders and interprets tests, and plans care for short-term and longitudinal needs.
- Ensures CKD/ESKD and heart failure care alignment across settings; supports transitions in care.
- Documents diagnoses to support risk adjustment and addresses care gaps.
- Presents patient cases and provides clinical support for rounds and meetings.
- Serves patients in-person and via telehealth or home visits; may include dialysis clinics.
