Strive Health is seeking a Clinical Lead Nurse Practitioner to guide care for patients with CHF, CKD/ESKD, and other chronic conditions. You’ll work across home, telehealth, and clinic settings with an interdisciplinary team to deliver longitudinal, patient-centered care.

You’ll leverage analytics and technology to coordinate treatment, reduce hospitalizations, and partner with external providers while maintaining board certification and state licensure.

Qualifikationen

• Master's degree in Nursing required.
• 2+ years of NP experience with cardiology (CHF) or nephrology (CKD).
• Current state licensure and NB certification.

Aufgaben

• Lead care for CHF, CKD/ESKD patients in home and telehealth.
• Collaborate with interdisciplinary teams to ensure care plan alignment.
• Manage a defined patient panel with quality, cost and outcomes focus.
• Document diagnoses for risk adjustment and care gaps.
• Support transitions across care settings and participate in team meetings.
• Maintain board certification and state continuing education requirements.
