Hackensack Meridian Health seeks an Advanced Practice Nurse (APN) or Physician Assistant (PA) for a hybrid outpatient nephrology/hypertension role in South River, NJ. The position emphasizes direct patient care, care coordination, and collaboration with a multidisciplinary team.

The role requires NJ licensure as an APN or PA and prescriptive authority per state law. On-call is required; dialysis rounds occur on occasional Saturdays, with a comprehensive benefits package offered.

Qualifications

• Graduate of NLN/AACN accredited nursing program.
• Master's or Doctorate degree from an accredited NP program.
• National Provider Identifier (NPI).
• Prescriptive authority through CDS and DEA registration according to New Jersey state law.
• Collaborative agreement with physicians on staff.
• Excellent written and verbal communication skills.
• Proficient computer skills including MS Office and Google Suite.

Responsibilities

• Performs and documents history and physical assessments appropriate for patient age and problem.
• Orders, performs, and interprets labs and diagnostic studies in collaboration with physicians.
• Managesgeneral medical and surgical conditions with knowledge of pathophysiology and pharmacology.
• Prescribes medications safely per hospital policies and NJ regulations.
• Mentors APNs, precepts students, and participates in educational programs.
• Participates in interdisciplinary rounds and coordinates care.
