Carease Health in OH is seeking an Adult Nurse Practitioner (NP) or Physician Assistant (PA) to deliver preventive care in skilled nursing facilities. The role combines on-site visits with remote work from home, supported by a dedicated scribe team for documentation.

You will use Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) to elevate patient care, build strong facility relationships, and help reduce hospitalizations.

Qualifications

• Active NP or PA license in the state of practice.
• 2+ years of experience in geriatrics or internal medicine.
• Excellent communication, collaboration, and teamwork skills.
• Ability to maintain patient confidentiality and adhere to HIPAA guidelines.

Responsabilités

• Provide patient-centered care to residents in skilled nursing facilities.
• Conduct on-site visits and remote work from home.
