Qualifications

• Active Adult Nurse Practitioner license in the practicing state.
• 2+ years of geriatric care and documentation review experience.
• Strong communication, collaboration and teamwork abilities.
• Ability to work independently and in a team-based setting.

Responsibilities

• Provide preventive care to residents in skilled nursing facilities with on-site and remote work.
• Develop and implement individualized care plans using RPM, CCM and CoCM.
• Assess, diagnose, treat acute/chronic conditions and interpret tests.
• Coordinate with healthcare teams for seamless care and communication.
• Maintain accurate records with support from a scribe team.
• Participate in quality improvement and assist in protocol development.
