Carease Health in Ohio is seeking an Adult Nurse Practitioner (NP) or Physician Assistant (PA) to deliver high-quality preventative care in skilled nursing facilities. This hybrid role combines on-site visits with remote work, supported by a dedicated scribe team for efficient documentation.

You'll implement Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) to reduce hospitalizations, improve patient outcomes, and support facilities and

Qualificações

• Active NP or PA license in the state of practice.
• 2+ years of experience in geriatric care or internal medicine.
• Excellent communication, collaboration, and teamwork skills.
• Ability to maintain patient confidentiality and adhere to HIPAA guidelines.

Responsabilidades

• Provide patient-centered care to residents in skilled nursing facilities.
• Conduct on-site visits and remote work from home.
