Carease Health - NJ is seeking a Nurse Practitioner to provide high-quality care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work, supported by a dedicated scribe team for efficient documentation.

The ideal candidate holds an active NP or PA license in the state, has 1+ year geriatric care experience, and excels in communication and teamwork to coordinate care with clinical staff and facilities.

• Active NP or PA license in the state of practice.
• 1+ year of experience in geriatric care.
• Excellent communication, collaboration, and teamwork skills.
• Ability to work effectively in a fast-paced environment.

Responsabilidades

• Build strong relationships with skilled nursing facilities and maintain a patient-centered approach.
• Deliver preventative care to residents, including Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) into our program.
• Collaborate with our clinical team to ensure alignment with facility processes and protocols.
• Provide excellent communication, collaboration, and teamwork to drive success in this role.
• Conduct on-site visits and remote work from home, supported by a dedicated scribe team for efficient documentation.
