Carease Health in Jackson, TN is seeking a dedicated Nurse Practitioner to join our team delivering high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a scribe team for efficient documentation.
You will work with our clinical team to integrate RPM, CCM, and CoCM into our care program, enjoy training from clinical directors, and benefit from generous PTO, free CEUs, and referral bonuses while
Qualifications
- Active NP or PA license in the state of practice.
- 1+ year of experience in geriatric care and documentation review.
- Excellent communication, collaboration, and teamwork skills.
Responsabilités
- Deliver high-quality preventative care to residents in skilled nursing facilities.
- Conduct on-site visits and remote work from home, supported by a scribe team for documentation.
- Build strong relationships with facility staff and maintain a patient-centered approach.
- Collaborate with our clinical team to provide comprehensive care to patients.
- Stay up-to-date with the latest medical research and guidelines to ensure patients receive the best possible care.
