Geode Health seeks a Licensed Mental Health Therapist for the Midlothian area. You will diagnose and treat mental health disorders, deliver psychotherapy, and coordinate with a care team to ensure consistent, evidence-based treatment.

The role offers a hybrid schedule with three days onsite and two days telehealth, allowing flexible patient access while maintaining full-time hours. You will contribute to outcome-based care and ongoing professional development.

Qualifications

• Active Texas licensure (LCSW, LCPC or LMFT).
• Experience conducting comprehensive assessments and developing individualized treatment plans.
• Proficiency in using an EMR system and maintaining accurate patient records.

Responsibilities

• Provide psychotherapy across the general spectrum of mental health conditions.
• Collaborate with the integrated care team to coordinate care.
• Deliver crisis intervention when needed and support care continuity.
• Partner with providers to move patients along the care continuum.
