Geode Health is seeking a Licensed Mental Health Therapist in Midlothian, TX to join our hybrid care team. You will diagnose and treat mental health disorders and provide psychotherapy across the spectrum, coordinating with other providers to deliver integrated care.

Qualifications

• Active license (LCSW, LCPC or LMFT) in the state of Texas.
• Experience conducting comprehensive assessments and developing individualized treatment plans.
• Proficiency in using an EMR system and ensuring accurate and up-to-date electronic health records.

Responsibilities

• Diagnose and treat mental health disorders and provide psychotherapy across the spectrum of care.
• Collaborate with a care team to coordinate services and ensure seamless patient care.
• Provide crisis intervention when needed and support patients across the care continuum.
