Responsibilities

• Provides grief counseling and crisis intervention for patients and families.
• Assesses discharge planning needs and coordinates post-acute care services.
• Reviews medical records to inform transition of care planning and discharge reconciliation.
• Develops discharge plans with contingency plans for timely care progression.
