AdvanceCare Health Services, LLC is seeking a dedicated Medical Coordinator to support our Medical Department with DDA/ECF/CLS experience. The role blends home-based work with weekly on-site duties and requires living within one hour of Hermitage, TN.

You will coordinate appointments, monitor plans, and assist individuals and families as part of a compassionate care team. The position offers a hybrid schedule, company-provided systems, and a focus on administration, documentation, and community

Qualifications

Experience as a medical coordinator or in DDA/ECF/CLS settings.
Ability to manage schedules, appointments, and patient records.
Strong communication and organization, capable of multitasking.
Must have reliable transportation and internet access.

Responsibilities

Serve as a medical case manager for individuals in the Family Model Program.
Monitor service plans in line with regulatory guidelines.
Coordinate appointments, meetings, and administrative tasks for individuals and staff.
Assist individuals and families in a supportive role.
Collaborate with other agencies on behalf of individuals served.
Assist in interviewing prospective individuals and families.
