Qualifications

Requires BA/BS degree and a minimum of 1 year of experience working directly with people related to the program population or other community-based organizations.
BA/BS degree field of study in health care related field preferred.
Must reside in Lenoir County and travel to worksite as needed.

Responsibilities

Manage member’s individual service plan within the scope of state law and contract.
Conduct telephonic or face-to-face history and program needs assessments.
Identify members with potential health care needs and coordinate with clinical teams to provide care coordination support.
Manage non-clinical needs of members with chronic illnesses to ensure cost-effective utilization of services.
Establish short and long-term goals with the member, caregivers, family, and physicians.
