Job summary

Qualifications

• Requires BA/BS degree and at least 1 year of relevant experience.
• BA/BS in health care related field preferred; state-specific requirements may apply.
• Willing to travel to work locations.

Responsibilities

• Manage the member's individual service plan within state law and contract.
• Perform telephonic or face-to-face history and program needs assessments.
• Coordinate cases with clinical team to provide care coordination support.
• Manage non-clinical needs of members with chronic illnesses, co-morbidities, and/or disabilities.
• Establish short and long term goals with member, caregivers, family, and physicians.
