Job summary

Fletcher Rehabilitation and Healthcare Center in North Carolina seeks a skilled MDS Coordinator for a hybrid role that combines three days on-site and two days remote. You will coordinate and manage resident care plans to ensure high-quality nursing care, while maintaining regulatory compliance and accurate documentation.

The ideal candidate holds an active RN license in NC, CPR certification, and has knowledge of MDS processes. Prior long-term care experience is preferred but not required.

Qualifications

• Active RN license in state of North Carolina.
• CPR certification required.
• Knowledge of MDS process preferred.

Responsibilities

• Keep abreast of current regulations and professional standards.
• Assist with development of comprehensive care plans in coordination with the MDS.
• Coordinate, manage and monitor the written plan of care for each resident.
• Monitor resident status changes and ensure timely involvement of nursing/clinical teams.
• Ensure MDS and support documentation accurately reflect residents and meet regulatory/auditor requirements.
• Perform audits of documentation for accuracy.
• Assist nursing management with discharge processes.
• Complete administrative forms and reports.
• Maintain infection control and standard precautions in care delivery.
• Follow safety precautions and properly use equipment and supplies.
• Maintain confidentiality of residents' health information.
• Report hazardous conditions, damaged equipment, and supply issues.
