Job summary

Fletcher Rehabilitation and Healthcare Center seeks an experienced MDS Coordinator for a hybrid role (3 days onsite, 2 days remote). You will coordinate and manage nursing care plans to ensure residents receive high-quality, compliant care.

Responsibilities include developing care plans with the MDS, auditing documentation for accuracy, guiding discharge processes, and ensuring adherence to infection control and safety standards while maintaining resident confidentiality.

Qualifications

• Current State License as a Nurse.
• CPR Certification.
• Experience in a long-term care setting preferred.

Responsibilities

• Coordinate, manage and monitor the written plan of care for each Resident.
• Assist with development of comprehensive care plans with the MDS.
• Conduct regular audits of documentation for accuracy and regulatory compliance.
• Assist nursing management with the discharge process.
• Maintain infection control practices and safety precautions.
• Maintain confidentiality of resident health information.
• Report hazardous conditions, equipment, and supply issues.
