Fletcher Rehabilitation and Healthcare Center is seeking a skilled MDS Hybrid Position to coordinate and manage nursing care plans, ensuring residents receive high-quality care. This full-time role blends onsite and remote work (3 days onsite, 2 days remote).

Qualifications

• Current State License as a Nurse.
• CPR Certification.
• Knowledge of MDS process and coordination.
• Preferred: 1 year experience as a nurse in long-term care.

Responsibilities

• Keep abreast of current federal and state regulations and standards.
• Assist with development of comprehensive care plans with the MDS coordination.
• Coordinate, manage and monitor the written plan of care for each resident.
• Monitor resident status changes for timely nursing or clinical involvement.
• Ensure MDS and support documentation meet regulatory and auditor requirements.
• Perform regular audits of documentation for accuracy.
• Assist nursing management with discharge processes.
• Complete necessary forms and reports.
