The Office of MaineCare Services (OMS) in Augusta, Maine, seeks a licensed social worker (LSW) to join as a Social Services Program Specialist focusing on complex case coordination for residents with disabilities.

The role is hybrid, based in Augusta with in-office meetings and remote work as required. You will coordinate care across systems, develop person-centered plans, and help clients access benefits and services.

Qualifications

• Six years of education and/or professional experience in social services.
• Current LSW credential issued by the Maine State Board of Social Worker Licensure.

Responsabilités

• Provide consultation and support to residents with disabilities on complex healthcare, financial, housing, and placement issues.
• Coordinate multidisciplinary team meetings for adults who do not have a safe discharge plan or face barriers to services.
• Coordinate efforts among healthcare providers, community agencies, state programs, and support systems to identify and implement solutions.
• Develop and support person-centered plans to promote safety, stability, and community integration.
