Job summary

MaineHealth is seeking a Social Worker (LSW) to join Lend A Hand within our Behavioral Health and Population Health programs. This full-time role provides complex care management in the primary care setting, addressing social drivers of health to help patients overcome barriers and reach health goals.

The position offers a day shift, ~40 hours/week, with hybrid work up to two days remote after training. Requirements include a Bachelor's in Social Work, Maine State LSW license, and experience

Qualifications

• Education: Bachelor's Degree in Social Work from an institution accredited by the Council on Social Work Education required.
• License/Certifications: Active license by the Maine State Board of Social Work required.
• Experience: Experience providing therapeutic services to adult and pediatric patients with chronic medical issues required. Experience in client advocacy and case management required.
• Additional Skills/Requirements: N/A

Responsibilities

• Provide complex care management in the primary care setting to support the Population Health program.
• Deliver professional social work services throughout the care tenure.
• Collaborate with care teams to address social drivers of health and barriers to goals.
• Maintain licensure and participate in care coordination and documentation.
