Job summary

Integrative Health Care Services of Indiana (IHCS) is seeking a Licensed Practical Nurse (LPN) – Care Coordination to support seniors and disabled individuals. The role blends nursing knowledge with nonclinical care coordination to help clients access services and remain independent in a hybrid work setting.

The LPN will work with clients, families, and providers to ensure timely visits, accurate documentation, and appropriate referrals.

Qualifications

• Current, active LPN license in Indiana.
• Valid driver's license and reliable transportation.
• Ability to travel regularly within assigned Indiana service region.

Responsibilities

• Developing, implementing, reviewing, and monitoring care coordination plans based on client needs and services.
• Conducting required face-to-face client visits and timely documentation.
• Identifying barriers to access and coordinating referrals, resources, and follow-up.
• Collaborating with healthcare providers, waiver case managers, caregivers, and community partners to support continuity of care.
• Advocating for clients and promoting independence and person-centered service delivery.
