Position Overview

At Integrative Health Care Services of Indiana (IHCS), our mission is to improve the quality of life for seniors and individuals with disabilities by providing compassionate, coordinated, and person-centered care.

The Licensed Practical Nurse (LPN) – Care Coordination serves as an advocate, educator, and trusted partner for clients by coordinating services, identifying barriers to care, and facilitating access to appropriate healthcare resources and community-based supports. This position combines the qualifications of a licensed practical nurse with the nonclinical care coordination responsibilities necessary to support clients in maintaining their independence, dignity, and quality of life.

The LPN works collaboratively with clients, families, healthcare providers, waiver case managers, caregivers, and community partners to facilitate appropriate services and support continuity of care. Responsibilities include client-facing visits, care coordination, communication, documentation, service follow-up, and compliance with applicable program requirements.

This is a nonclinical care coordination position. IHCS does not provide direct clinical treatment through this role. The LPN will use their professional knowledge to support service coordination, recognize concerns that require referral or escalation, and facilitate communication with appropriate healthcare professionals. The position does not authorize the employee to perform activities outside IHCS's approved service scope or applicable regulatory requirements.
