CareHarmony is seeking an experienced Licensed Practical Nurse (LPN) in Indianapolis, Indiana, for a fully remote position. The role involves conducting health assessments, resolving patient inquiries, and managing chronic care. Qualifications

• At least 3 years of direct patient-facing work experience.
• Must have active high-speed Wi-Fi.
• Must have a home office or HIPAA-compliant workspace.

Responsibilities

• Conduct preliminary health assessments for newly enrolled patients.
• Resolve questions and create dialogue to understand patient needs.
• Coordinate community resources that would benefit patient care.
• Provide patient education and health literacy.
• Assist with medication management and coordinating refills.
• Ensure timely delivery of services to patients.
