Job summary

UPMC Montefiore/Liver Transplant is seeking a dedicated Registered Nurse to join as Clinical Transplant Coordinator. This role supports patients through the transplant journey, from referral to post-transplant care, with a mix of in-office and remote work on a 8am-4pm schedule.

You will coordinate evaluations, educate patients and families, and collaborate with a multidisciplinary team to ensure regulatory compliance and high-quality outcomes. ABTC certification is encouraged.

Qualifications

• Active RN license required.
• BSN or higher degree.
• Experience in transplant nursing preferred.
• Ability to collaborate with a multidisciplinary team.

Responsibilities

• Coordinate evaluation process for transplant candidates with the team.
• Provide patient and family education and maintain transplant listing status.
• Assist with peri-operative coordination and admissions/transfers.
• Monitor post-transplant patients and adjust care plans as needed.
• Pursue ABTC certification and participate in ongoing education.
