AdventHealth in Orlando, FL seeks a transplant coordination professional to organize appointments, evaluate eligibility for advanced therapy, and liaise between patients, families and a multidisciplinary medical team. The role emphasizes education, records management, and ongoing support through post-treatment care.

You will monitor progress, facilitate referrals, and ensure adherence to prescribed therapies while contributing to quality improvement initiatives within a Hospital setting.

Qualifications

• Ability to communicate verbally and in written form in English with physicians, patients and staff.
• Strong interpersonal, organization skills and ability to work with interruptions.
• Autonomous, self-directed with high complexity and intensity.
• Self-motivated with the ability to multi-task and adapt.
• Maintain confidentiality per HIPAA/Florida guidelines.
• Knowledge of clinical documentation, exam procedures and protocols.
• Strong communication and organizational skills.
• Basic clerical skills and typing 20 WPM.
• Bilingual (English required/Spanish preferred).
• Medical terminology preferred.

Responsibilities

• Organizes and schedules appointments with transplant teams, surgeons, and cardiologists.
• Assesses patient eligibility for advanced therapy or organ transplantation.
• Coordinates initial referral processing and patient evaluations.
• Provides detailed education to patients and families about treatment and care.
• Maintains accurate records of patient interactions and care plans.
• Acts as liaison between patient, family and healthcare team.
• Monitors progress post-treatment and schedules follow-ups.
• Ensures medication adherence and manages side effects.
• Collaborates to develop comprehensive care plans.
• Supports patients and families with emotional and mental health resources.
