Job summary

Qualifications

• Requires MS/MA in social work or nursing degree with clinical experience in behavioral health.
• Active, current license to practice as RN, LCSW, LMHC, LPC, LMFT, or clinical psychologist.
• Experience with foster care, displaced or adopted children is preferred.

Responsibilities

• Conducts assessments to identify individual needs.
• Develops comprehensive care plans addressing identified objectives.
• Supports member access to quality, cost-effective care.
• Coordinates with internal and external resources for integrated care.
• Addresses social determinants of health in care planning.
• Provides patient education, medication reconciliation, and care follow-up coordination.
