Job summary

Centene is hiring for a Hybrid Behavioral Health Case Manager based in Austin, TX. You will develop and coordinate care plans for members with complex mental/behavioral health needs, educating families and coordinating with providers to ensure effective services.

The role requires a Master’s degree in Mental Health or Social Work (or Nursing) and 2–4 years of related experience, with up to 75% travel for home visits. Licensure as BH professional or RN with BH experience is required.

Qualifications

• Requires a Master’s degree in Mental Health or Social Work or Nursing and 2–4 years of related behavioral health experience.
• Licensed Behavioral Health Professional or RN with BH experience per state requirements is required
• Experience collaborating with medical and behavioral health providers and managing community resources.

Responsibilities

• Evaluate needs of high‑risk members with mental/behavioral health needs and develop care plans.
• Act as liaison between member/family, physician, and facilities.
• Provide education to members and families on services and benefits.
• Perform frequent home visits (up to 75% travel) to assess needs and coordinate resources.
• Maintain accurate documentation in the care management system and ensure compliance with guidelines.
• Travel for member visits is required.
