Job summary

Elevance Health is seeking a Telephonic Nurse Case Manager I to perform care management for members with complex needs. The role requires assessment, planning, coordination, and monitoring of care plans across the continuum, with licensure and multi-state capability.

The position is remote with in-person training sessions, requires GA residency and multi-state licensure, and offers a structured schedule Mon–Fri with EST shifts. A pre-employment skills assessment applies.

Qualifications

• Requires a BA/BS in a health-related field.
• 3 years of clinical experience; or any combination of education and experience, which would provide an equivalent background.
• Current, unrestricted RN license from the State of Georgia required.
• Multistate licensure is required if this individual is providing services in multiple states.

Responsibilities

• Telephonic care management within licensure scope for members with complex and chronic care needs.
• Assess, develop, implement, coordinate, monitor, and evaluate care plans to optimize member health care.
• Ensure member access to services appropriate to health needs.
• Conduct assessments to identify needs and create care management objectives.
• Coordinate authorizations/referrals within benefits or extra-contractual arrangements.
• Interface with Medical Directors and Physician Advisors on care plans.
