Premise Health Inc. in Georgetown, KY is seeking a Full Time Leave Coordinator to support client employees from our Health Center.

The role offers a hybrid schedule (Monday-Thursday onsite, 8:00 AM-4:30 PM) with Friday work from home, providing care and coordination for disability absences and related needs. You will liaise with employees, providers, HR, and administrators, ensure compliance with laws, maintain records, and assist with health events.

Qualifications

Minimum 2 years in Human Resources preferred.
Certified professional in disability management (CPDM) preferred.
A minimum 2 years of experience in absence management (leaves, ADA, Workers’ Compensation).
High school degree required; Bachelor's degree preferred.

Responsibilities

Monitor, track, coordinate, and administer attendance and altered work schedules for disability.
Coordinate work accommodations, workers’ compensation, fitness for duty, and state/federal leaves.
Serve as liaison between employees, providers, HR, and third-party administrators.
Ensure compliance with federal and state laws and company policy.
Maintain records, reports, and forms; perform follow-up as directed.
Support health and wellness events and partner programs.
Escalate duties beyond administrative scope to clinical team as needed.
