Job summary

Premise Health is seeking a Leave Coordinator to support client employees at our Princeton, IN health center. This full-time, hybrid role combines onsite days Monday-Thursday with a Friday work-from-home option.

You will coordinate absences and accommodations, ensure regulatory compliance, and collaborate with HR, vendors, and clinical staff. This role requires 2+ years in HR and experience with absence management; CPDM is preferred.

Qualifications

• High school degree required; Bachelor's preferred.
• Minimum of 2 years in Human Resources preferred.
• Certified professional in disability management (CPDM) preferred.
• Minimum 2 years of absence management experience (leaves, ADA, Workers’ Compensation).

Responsibilities

• Monitors, tracks, coordinates, administers work absence and altered work schedule for disability.
• Coordinates work accommodations, workers’ compensation claims, fitness for duty, state, federal and company leaves.
• Primary liaison between client employees, providers, agency or institution, case managers, client’s HR, Corporate Health, benefits regarding the employee’s absences or altered work schedule.
• Ensures compliance with all federal and state laws and company policy.
• Assists in maintaining records, reports, forms and performs follow-up as directed.
• Aids in coordination of client health and wellness events, programs through client’s vendor partner.
• Looks for ways to improve and promote quality; demonstrates accuracy and thoroughness.
• Appropriately escalates duties beyond their administrative scope to clinical team.
• May require other duties as assigned.
• A liaison between client employees and third party administrators.
• Provides oversight to the 300A, BLS Survey and Client’s Manhours in tandem with the Client's Safety Management Team
