Job summary

Heritage Health Network in Orange County seeks a Lead Care Manager to support members in the Enhanced Care Management (ECM) program. This hybrid role combines field visits with remote coordination.

You will develop personalized care plans, collaborate with healthcare teams, educate and advocate for members, and address gaps in care while maintaining thorough documentation.

Qualifications

• Experience in care or case management (community-based preferred)
• Knowledge of Medi-Cal and ECM strongly preferred
• Strong communication and interpersonal skills
• Ability to work in a team-oriented environment
• Willingness to travel within Orange County
• Bilingual (English/Spanish) preferred
