Job summary

Heritage Health Network (HHN) is seeking a Lead Care Manager to support members through our Enhanced Care Management (ECM) program in a hybrid role. You will be the primary point of contact, coordinating care and developing personalized plans with providers while collaborating with healthcare teams and community resources across Orange County.

Candidates should have care or case management experience, knowledge of Medi-Cal/ECM, and strong communication skills.

Qualifications

• Experience in care or case management (community-based preferred).

This is a hybrid position (2 days remote, 3 days in the field), giving you the flexibility to work from home while building strong, in-person connections with members across Orange County.
