Job summary

Heritage Health Network (HHN) invites you to join as a Lead Care Manager to support members through the ECM program in a hybrid role. You will travel within the Low Desert region and serve as the primary contact, coordinating care and developing personalized plans with members and providers.

Collaborate with healthcare teams, provide education and advocacy, and address gaps in care while maintaining timely documentation. This is a growth-focused role with a supportive team and community impact.

Qualifications

• Experience in care or case management (community-based preferred).
• Knowledge of Medi-Cal and ECM strongly preferred.
• Strong communication and interpersonal skills.
• Willingness to travel within Orange County.

Responsibilities

• Serve as the primary point of contact for members and coordinate their care.
• Develop and manage personalized care plans with members and providers.
• Collaborate with healthcare teams and community resources.
• Support members through education, advocacy, and engagement.
• Identify and address gaps in care.
• Maintain accurate, timely documentation.
