Job summary

Old Colony YMCA is seeking an Information & Referral Specialist to provide information to consumers in response to service and eligibility inquiries. The role supports older adults and individuals with disabilities, ensuring access to information and coordination of services.

You will assess health and functional status, develop care plans, coordinate services, and participate in care planning for community-based supports, including determining needs for long-term care.

Qualifications

• Bachelor's degree in social work or related field preferred; licensure if applicable.
• Minimum two years' professional experience caring for people 65 and older.
• Strong written and verbal communication; ability to interview clients.
• Ability to work effectively in an interdisciplinary team.
• Proficient with Microsoft Office and related software.

Responsibilities

• Assess health and functional status of members.
• Develop community-based care plans and service packages.
• Contribute to a member's ICP and care coordination.
• Arrange, coordinate, and monitor provision of services.
• Assist in determining need for institutional and community long-term care.
