Northeast Florida Area Agency on Aging Inc seeks an Information and Referral Specialist to provide guidance on community services for elders, people with disabilities, and caregivers. You will triage calls, document data, and refer clients to Medicaid, Medicare, LTC, and DOEA programs in a high-volume helpline setting.

The role requires a High School Diploma with at least two years of I&R experience, strong communication skills, and strict adherence to Inform USA Standards and privacy

Formación

• High School Diploma and at least two years in information and referral services or related work.
• Commitment to Inform USA Standards and client confidentiality (HIPAA).
• Strong written and verbal communication, plus data entry accuracy.

Responsabilidades

• Answer the Aging and Disability Resource Center Helpline in a fast-paced environment.
• Provide information and referrals to Medicaid, Medicare, LTC, and DOEA programs.
• Record caller data in the information and referral system and document outcomes.
• Schedule appointments and assist callers with referrals and benefits applications.
• Advocate for clients and follow up on referrals to determine outcomes.
