Job summary

Qualifications

• Bachelor's degree in Social Work, Sociology, Psychology, Healthcare or related field - Required
• 2 Years human services or related field - Required
• Certification provided by the Department of Community Health is required to be completed within 60 days from the hire date.

Responsibilities

• Performs comprehensive assessments geared towards developing care plans and quarterly reviews.
• Demonstrates ability to identify needs and coordinate care with providers to ensure adherence to care plans.
• Maintains timely and accurate documentation and reports to support service coordination and policy development.
• Meets with supervisor monthly to review cases and progress.
