• Bachelor's degree in social work or related field required.
• At least 2 years of case management experience required.
• Experience with home study development preferred.

Aufgaben

• Document calls, emails, and tasks using the CRM.
• Ensure family contact within 24 hours and plan visits within 30 days.
• Conduct thorough assessments and inspections of homes.
• Communicate concerns with Lead and Director.
• Follow safety guidelines and HIPAA requirements.
• Attend meetings and report ethical or legal issues.
• Perform other related duties as assigned.
