Job summary

Matter of Care, part of CenterLight Health System, seeks an experienced Clinical Assessment Manager to conduct in-home assessments and determine care needs for older adults in Brooklyn and lower Manhattan. The role emphasizes clinical collaboration with enrollment, intake, care delivery, and IDT teams to optimize services.

Qualifications

• Current, active, unrestricted New York State Registered Nurse license.
• Minimum of 5 years of clinical nursing experience.
• Experience in managed care, PACE, MLTC, home care, case management, utilization management, population health, or related healthcare settings.
• Strong clinical judgment, communication, and documentation skills.
• Ability to travel throughout Brooklyn and lower Manhattan.

Responsibilities

• Conduct comprehensive in-home clinical assessments utilizing the UAS-NY.
• Evaluate participant eligibility and ongoing care needs for community-based long-term care services.
• Perform reassessments and identify changes in condition, functional status, diagnoses, and medication regimens.
• Develop proposed service plans and recommendations to support participant health and independence.
• Provide hands-on services in participants’ homes as needed, including administering medications, wound care, and other clinical procedures within scope of practice.
