Job summary

Matter of Care, part of the CenterLight Health System family, is hiring a Clinical Assessment Manager to lead in-home assessments in Brooklyn and lower Manhattan. You will evaluate eligibility, develop service plans, and coordinate with care teams to support independent living.

The role requires an active NY RN license and 5+ years of nursing experience. Expect a Monday–Friday schedule with field visits and some remote work after training.

Qualifications

• Active NYS RN license.
• Minimum of 5 years of clinical nursing experience.
• Experience in managed care, home care, or related settings.

Responsibilities

• Conduct in-home clinical assessments using UAS-NY.
• Evaluate eligibility and ongoing care needs for community-based LTC.
• Perform reassessments and identify changes in condition, function, and meds.
• Develop proposed service plans and recommendations for independence.
• Provide hands-on services in homes as needed (med administration, wound care).
• Collaborate with Enrollment, Intake, Care Delivery, and IDT teams.
• Document clinical findings promptly in EMR systems.
• Participate in quality improvement and act as a clinical resource.
