Carease Health - PA is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to deliver preventive care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work, supported by a dedicated scribe team to ensure efficient documentation.

Candidates should have geriatrics or internal medicine background, an active NP/PA license, and strong communication and teamwork skills.

Competenze

• Active NP or PA license (in the state of practice).
• 1+ year of experience in Geriatric care.
• Excellent communication, collaboration, and teamwork skills.

Mansioni

• Provide high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote work from home.
• Conduct patient assessments, develop treatment plans, and provide education to patients and families.
• Collaborate with facility staff to ensure seamless care transitions and comprehensive patient care.
• Monitor patient progress, identify trends, and adjust care plans accordingly.
