Carease Health seeks a Nurse Practitioner or Physician Assistant to provide preventive care in skilled nursing facilities through a hybrid on-site and remote model. You’ll work with a scribe team, build relationships with facilities, and focus on patient-centered care while coordinating with the clinical team.

Experience in geriatrics or internal medicine is required; active NP/PA license and strong communication are essential. A six-figure salary and work-life balance are offered.

Qualificações

• Active NP or PA license in the state of practice.
• 1+ year of experience in geriatrics or internal medicine.
• Excellent communication, collaboration, and teamwork skills.

Responsabilidades

• Provide high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote work from home
• Conduct patient assessments, develop treatment plans, and provide education to patients and families
• Collaborate with facility staff to ensure seamless care transitions and comprehensive patient care
• Monitor patient progress, identify trends, and adjust care plans accordingly
• Maintain accurate and timely documentation, supported by a dedicated scribe team
