Qualifications

• Active NP or PA license in the state of practice.
• 1+ year of experience in geriatric care or internal medicine.
• Strong communication, collaboration, and teamwork skills.

Responsibilities

• Deliver preventative care to residents in skilled nursing facilities.
• Mix on-site visits with remote care supported by a scribe team.
• Build strong relationships with facilities and maintain patient-centered care.
• Collaborate with clinicians and care coordinators to ensure coordinated care.
