Job summary

Qualifications

• Must have a strong background in case management and care coordination.
• Experience conducting in-home assessments and visits.
• Ability to work effectively with adult populations with complex medical, behavioral, and social needs.
• Willingness to travel daily and perform field work.
• Valid driver's license and reliable transportation.

Responsibilities

• Establish relationships with internal teams and partners; educate on resources.
• Manage an active caseload per state guidelines; track progress monthly.
• Perform research, analysis, and reporting; update service plans.
• Lead the Person-Centered Service Planning (PCSP) process.
• Assist Members in accessing LTSS, Medical Assistance, and non-Medicaid services.
• Inform Members about LTSS options and fair hearing rights.
• Collect information to inform PCSP development and reevaluate care levels.
• Coordinate with providers to maintain LTSS eligibility.
• Ensure compliance with regulations and HIPAA; protect confidentiality.
• Work in the field ~75% of the time and independently in a virtual setting.
