Valid8 Financial, Inc. seeks a Service Coordinator to work with families of children ages 0-3, providing service coordination, developing the IFSP, and monitoring funded service delivery.

The role requires coordinating with A&I Avenues and other providers to ensure appropriate supports and documenting interactions in the health records. The position emphasizes advocacy, crisis negotiation, and ensuring compliance with federal and state guidelines.

Qualifications

• Bachelor’s degree in human services-related field is highly preferred.
• Five years’ work/volunteer experience with long-term care and/or disabilities programs and services.
• Some combination of relevant education and experience appropriate to the requirements of the position.

Responsibilities

• Develops and implements the IFSP.
• Coordinates and monitors delivery of services and supports.
• Advices families on service options and assists in acquiring needed services.
• Coordinates the delivery of services and supports from A&I Avenues and other organizations providing services/supports to the child and/or their family.
• Maintains regulatory compliance and ensures documentation for funding.
• Contacts families monthly and documents the contact in electronic health record.
• Completes Targeted Case Management billing monthly with timely, accurate case notes.
• Ensures all required documentation is in accordance with applicable Federal, State, and A&I Avenues rules and regulations.
• Leads the IFSP team in periodic and annual reviews and communicates with the family throughout the year.

Education
