Qualifications

Responsibilities

• Serve as a family system navigator to support families in I/DD and MH system navigation needs.
• Coordinate with UNC pediatric providers, NC START, and other key agencies to link families to services.
• Document needs and outcomes; track referrals and progress in EHR (Epic) and other tools.
• Participate in weekly team meetings, provide education and guidance to practice staff.
