Job summary

Qualifications

• New York State Licensed Registered Nurse (RN).

Responsibilities

• Provide person-centered nursing care with small, manageable caseloads.
• Coordinate medical care, appointments, and follow-up for individuals with I/DD.
• Oversee medication administration and ensure regulatory compliance.
• Advocate for people supported within healthcare and clinical settings.
• Collaborate with interdisciplinary teams on individualized care plans.
• Maintain accurate health records and nursing documentation.
• Support and educate staff on health, safety, and medical best practices.
