Job summary

Kalaya’s Destiny is seeking an ECM Lead Care Manager to oversee care planning and direct services in a hybrid setting across Los Angeles, Orange, Riverside, and San Bernardino counties. You will coordinate care, advocate for clients, and apply trauma-informed care while partnering with clinicians and community organizations.

The role offers a part-time starting path to full-time with benefits upon transition; compensation includes an hourly rate of $20.00–$25.00 and potential full-time salary

Qualifications

• At least 3 years of relevant work experience.
• High school diploma or GED required.
• Long-time resident with knowledge of local resources.

Responsibilities

• Assess client needs across physical health, mental health, substance use, oral health, trauma-informed care, social supports, housing, vocational/employment, and wellness.
• Develop and oversee implementation of the Individual Care Plan / Health Action Plan.
• Deliver services where the client lives, seeks care, or finds most accessible (office, telehealth, or field-based).
• Connect clients to needed social services and community supports.
• Coordinate and advocate with the client’s other health care providers with permission.
• Apply evidence-based practices: Motivational Interviewing, Harm Reduction, Trauma-Informed Care.
• Collaborate with hospital staff on Transitional Care Planning.
• Conduct outreach and engagement to enroll members in ECM.
• Accompany clients to office visits as needed.
• Document progress and attend team meetings and trainings.
