Job summary

Heritage Health Network is seeking a Lead Care Manager to oversee Enhanced Care Management (ECM) for members. This hybrid role combines field visits across Los Angeles County with remote planning and coordination.

You will develop personalized care plans, connect members with providers and community resources, educate and advocate, and maintain thorough documentation to ensure seamless care delivery.

Qualifications

• Experience in care or case management (community-based preferred).
• Knowledge of Medi-Cal and Enhanced Care Management (ECM) preferred.
• Strong communication and interpersonal skills.
• Ability to work in a team-oriented environment.
• Willingness to travel within Los Angeles County.
• Hybrid work flexibility.

Responsibilities

• Serve as the primary point of contact for members and coordinate their care.
• Develop and manage personalized care plans with members and providers.
• Collaborate with healthcare teams and community resources.
• Support members through education, advocacy, and engagement.
• Identify and address gaps in care.
• Maintain accurate, timely documentation.
