Job summary

The North Carolina Department of Health and Human Services is seeking an Early Intervention Service Coordinator (EISC) at the Sandhills CDSA to coordinate services for birth to three children with developmental delays.

You will partner with families and multidisciplinary teams to develop IFSPs, conduct home visits, arrange therapies, and plan transitions at age three, with telework options and a hybrid schedule.

Qualifications

• Bachelor’s degree and two years of professional experience with the target population.
• Ability to work with birth to three families and multidisciplinary teams.
• Willingness to drive across the CDSA catchment area.

Responsibilities

• Develop functional IFSPs with families and team members.
• Work with multi-disciplinary teams to determine needed supports.
• Conduct home visits to support families and children.
• Coordinate services such as speech and physical therapy.
• Arrange and monitor other early intervention services.
• Plan for transition to other services at age three.
